Comparison of Cortical and Medullary Bone Density at the Level of the Lower First Molar Using Cone Beam Computed Tomography

Summary
This study analyzes the morphology of bone density at the level of the mandibular first molar, evaluating its distribution in the vestibular cortical, medullary, and lingual regions, based on differences between genders and skeletal classes. Despite the clinical relevance of these morphological variations, the current literature provides limited detailed information on these differences. An analysis was conducted on 120 cone beam computed tomographies (CBCT) of adult patients, equally categorized by gender and skeletal class (I, II, and III). Measurements were taken in Hounsfield units, assessing the morphological characteristics of the three bony regions of the molar. Significant variations in bone density and morphological structure were identified between skeletal classes, with the vestibular cortical exhibiting greater density in class III males, while in females, the most pronounced differences were observed in the vestibular cortical between classes I and III. The morphological arrangement of bone density reveals specific patterns in each gender and skeletal class, which has important implications for dental biomechanics and clinical procedures such as implant placement. The morphological analysis of bone densities provides a better understanding of the structural differences in the mandible, potentially leading to more precise therapeutic strategies.
